I've had a lot of luck using Skype Conferencing for small video/audio conferences.

I have a list of <a href="http://cheapconferencecallingcompanies.com" rel="nofollow">cheap conference calling companies</a> too.

Video conferencing must be clear and high quality for sign language to be viewed clearly instead of blurring. It must be as clear as your regular TV news anchorpersons or weather guys, you know what I mean? They can freeze the viewer while dedicating full assymnetrical flow to the signer. Then the signer can press a floor pedal to switch to other end when he is done with signing and be frozen in a picture. It is like walkie talkie. This way high quality video can be flowed one way by borrwoing the other way back temporarily and vice versa as both ends switch turns.
